Glycation and why you care

What is glycation? In short glycation is a scientific term for what happens when sugars, such as fructose or glucose, combine with proteins or lipid molecules. Additionally, glycation is the process bakers and cooks refer to as browning. You may be shocked to learn that this browning actually happens to us as we age. From an anti-aging perspective this is not a good thing.With this information it is easy to see that we have two forms of glycation that we need to be concerned about. We have Endogenous glycations, which predominately occur in the bloodstream and exogenous glycations that occur out side the body.Advanced Glycation Endproducts (AGEs) are formed when sugars are cooked with proteins and fats. What you may not realize is that very often sugars are added to many of our foods specifically to enhance this browning effect. AGEs have been implicated in many age-related chronic diseases such as: Type II Diabetes, Cardiovascular disease, Alzheimer's disease, Cancer, Neuropathy and the list goes on.Did I mention that Glycation and AGEs have a very important partner in the damage of our tissues? As you may imagine the flavanoids found in fruits, vegetables, and whole grains have been found to reduce the glycation of hemoglobin.In conclusion, while glycation, AGEs and their partner in crime, free radicals, lead to the initiation of many of our age related diseases we have the ability to avoid being plagued by these problems. In fact it is really more than a choice on our part. If you have watched anyone in their later years and thought to yourself that you wanted a different path: you have that option. The downside to that choice is that it is not about making that decision for a single day. What you have to commit yourself to is a lifestyle change. Understanding Diabetes

So you or someone in your family has just been diagnose with diabetes. Maybe you or your family member has been living with diabetes for several years now. Just want to let you know I feel your pain on a personal level. A family member of mind were diagnose 5 years ago with the disease.What exactly is diabetes well I will cover the basics with you. What actually causes type 1 diabetes is not really known. Doctors believe that it might be genetic. Now type 2 diabetes on the other hand is a little different. Type 2 diabetics can control their blood glucose by taking oral medication and managing their diets. That is not the case for all type 2 diabetics so have to control their diabetes with insulin injections. Taking oral medication is not an option for type 1 diabetics instead they are dependent upon insulin injections. One thing I would like to say is not to believe everything you hear. As was mention earlier, the rumor that was taught to me was that eating to many sweets will cause you to have diabetes. That is definitely proven to be a myth for type 1 diabetes. My son was one and he didn�"t even have a chance to start eating foods that was high in glucose. Not here to preach; know that the doctor provided you with enough education at the time of diagnosis.

Understanding the complications of DKA

Diabetic Ketoacidosis is the most severe form of dehydration for an insulin dependent diabetic (Type I) but also in the non-insulin dependent diabetic (Type II experience HHS). Also know as DKA (Diabetic Ketoacidosis) it can occur from a relative or absolute deficiency of insulin; demonstrated by hyperglycemia, dehydration, and acid producing derangements of metabolism; acidosis. This condition can present as a true emergency in the insulin dependent diabetic or to the person that did not know they had diabetes; only signs and symptoms of dehydration or malaise. The causes of DKA and hyperglycemia can result from infection, disruption of treatment, and as a result of new onset; typically characterized by having a blood sugar (BS) above 300mg/dL, low bicarbonate (<15mEq/L), and acidosis with a pH less than 7.30 and associated ketonemia and ketonuria. Some of the complications from DKA can be cerebral edema (the major cause of mortality and morbidity in DKA), renal failure, cardiac disturbances, pulmonary edema, or even acute gastric dilation.In order to understand how some of the signs and symptoms occur you need to look into a basic physiological process. The body uses many mechanisms to balance acid levels, energy levels, energy sources, and function which is termed maintaining homeostasis. When homeostasis is jeopardized the body tries to counteract and protect itself. However, when our internal mechanisms cannot maintain homeostasis any longer it requires medical treatment and intervention based on clinical (lab and diagnostic) studies. In the diabetic, more specifically DKA, conditions such as a silent MI, CVA, or thrombosis may contribute to the evolvement of DKA.The primary mechanism behind DKA occurs when insulin is deficient, levels of glucagons elevate, catecholamines are stimulated and hepatic glucose production is increased. The response to this is a decrease in the uptake of insulin from the peripheral tissues. The bottom line is that there is elevated production of glucose and non-usage of the circulating peripheral glucose.Insulin is the primary anabolic hormone which means that muscle, fat and the liver do not take up glucose. There are other hormones (counter regulatory) such as glucagon, growth hormone, and catecholamines which accelerate the triglyceride breakdown into free roaming fatty acids and stimulate gluconeogenesis. This causes an elevation in the blood glucose levels in DKA. A result of the oxidation of free fatty acids (in the liver) is the production of ketone bodies, the two primaries are beta hydroxybutyrate and acetoacetic acid, hence the term switching metabolism; changing from carbohydrate metabolism to fat metabolism.During the metabolic mayhem, a metabolic acidosis prevails. This condition of acidosis depletes the fluid between and inside the cells of its acid buffers. As the body begins to lose its ability to buffer acidic conditions adequately it finds an alternate route to eliminate the ketones; via the urine (ketonuria). Subsequently when blood concentrations of glucose are exceedingly high, the ability of the kidney to absorb glucose is exceeded and the glucose begins to spill into urine waste (glycosuria). The resulting complication here is free water loss because of a condition known as osmotic diuresis which results in dehydration, thirst, low perfusion to tissues, and quite possibly lactic acidosis. This would explain a common set of symptoms called Poly, Poly, Poly⬦..Polyuria, Polydipsia, and Polyphagia.Polydipsia comes from dehydration that is sensed by the hypothalamus because you are thirsty from the loss of water. Polyuria occurs because you are urinating so much from the body�"s attempt to rid itself of the excess glucose and buffer the acidity of the blood. Polyphagia comes from the switching of metabolism, the body is requiring carbohydrates for energy and you just cannot use the energy source; a lack of insulin.With the loss of free water and the existence of changes in the osmotic gradient the patient is at severe risk for a severe electrolyte imbalance. The primary electrolyte concerns are sodium, potassium, magnesium, and phosphate. Potassium loss occurs as a result of extraction from intracellular fluid to exchange for the hydrogen ion build up in the extracellular areas in order to help with the buffering process. The potassium loss continues through the urine because of the change in the osmotic gradient. Sodium is lost in a similar fashion but the initial hyponatremia present in evaluation is related to a dilutional process. Water is extracted into the extracellular space as well, creating the dilution and is lost via urinary excretion due to the osmotic gradient change. Magnesium and phosphorous are lost similarly. The most important of the mentioned electrolytes is potassium. The production of the ketone bodies, in abundance, will lead to general abdominal pain with nausea and vomiting contributing to further loss of sodium and potassium.The goals of treatment are to reverse the rehydrate the body, correct the hyperglycemia, and replenish the loss of electrolytes. After initial identification and treatment you must find the underlying cause and further treat any of the potential complications; MI, CVA, cardiogenic shock, sepsis, and thrombus. IV fluid regimens will vary upon diagnostic and lab evaluation and ensure proper renal function. This would also be true for the levels of electrolyte losses. Additional complications will be treated as they are discovered.In conclusion the patient with Type I diabetes or new onset of diabetes can be life threatening and is considered a true medical emergency. Early identification and intervention are necessary to prevent complications. Early identification consists of not only correcting the condition of hyperglycemia, hypovolemic, and electrolyte imbalances but more importantly the underlying cause. These causes can range from infection to organ dysfunction. Close monitoring of treatment effectiveness can reduce or eliminate the long term complications of cardiac disruption, sepsis, hypovolemic shock, and renal damage; consequences of cerebral edema also need to be considered.

Every Person Before Developing Type 2 Diabetes Almost Always Has Pre-diabetes

What is Pre-diabetes? This is a fact that before developing diabetes mellitus type 2, almost always that patient develops Pre-diabetes condition. Though we will not diagnose that person as diabetic, but if he or she will not control his or her blood suger level, he will probably have diabetes mellitus in future. In pre-diabetes, blood sugar levels are in higher limits, but not yet high enough to be diagnosed as diabetes mellitus. In USA only, more than 54 million people have pre-diabetes condition.During pre-diabetes damage to heart and circulatory system may already be occuring. Various studies have shown that if you control your pre-diabetes condition earlier, then there are very less chances to develop type 2 diabetes mellitus, or you can delay it to develop. So always try to manage your blood glucose under control limits when you are in pre-diabetic stage to prevent yourself from developing type 2 diabetes. Do you have Pre-diabetes Condition, How to Tell? There are two important tests that can tell you that you are having pre-diabetes condition. 1-The fasting plasma glucose test (FPG) 2: Oral glucose tolerance test (OGTT). Your doctor can do any of these two tests. Fasting Plasma Glucose Test values for pre-diabetes are between 100mg/dl to 125mg/dl. If your glucose level is below 100mg/dl, then you have normal metabolism, if above 125mg/dl then you have Diabetes. It means if your level is between 100-125mg/dl, then you will have pre-diabetes. In case of OGTT, if less than 140mg/dl, then normal, between 140-200mg/dl is pre-diabetic condition and above 200mg/dl is diabetic condition. How you can prevent pre-diabetes? Pre-diabetes can be very lethal to your health if you will not manage it seriously. Sooner or later you will probably have type 2 diabetes. There are two simple things that you can do to get rid of this pre-diabetes situation. You can also even get your glucose level within normal limits if you follow these 3 steps. A: Change in your diet habit. Try to avoid sweets in your diet. B: Half hour moderate exercise C: Weight reduction. Just 30 minutes daily moderate physical activity or exercise, along with a 5-10% reduction in body weight, can produce a 58% reduction in diabetes Who should get tested for pre-diabetes? If you are overweight and above 45 years of age, then you should have your FPG and OGTT. If you are not overweight and above 45, should consult your doctor. If you are below 45 and overweight, you should also check your fasting plasma glucose level. How often should I be tested? If you are having normal blood glucose levels you should have FPG test every 3 years. If you have pre-diabetes, you should be checked for type 2 diabetes every year after your diagnosis of pre-diabetes. Children pre-diabetes Children have much less chances of developing pre-diabetes condition, however if they are very obese then one must think of pre-diabetes condition and get yourself be checked for pre-diabetes.

What Are Normal Blood Glucose and Blood Sugar Levels

This is the question my diabetic patients always ask. There are various methods to determine blood glucose level. Some tests give you accurate diagnosis of diabetes or pre-diabetes, while others will tell you how well you are managing your diabetes.Fasting Blood Sugar Test:Measures the blood sugar level after 8 hours fast or overnight. Normal fasting blood glucose level is less than 100mg/dl. If your fasting blood glucose level is from 100mg/dl to 125mg/dl then you will have impaired blood glucose level also known as Pre-Diabetes. If your blood glucose level is above 125mg/dl then your doctor will diagnose as a patient of diabetes. To confirm the diagnosis, your doctor may repeat the fasting blood glucose test on any other day. If you have blood glucose level of 126mg/dl or higher in two consecutive tests, then you may have diabetes. If you have blood glucose level greater than 200mg/dl and you have symptoms of diabetes like increased thirst or hunger, frequent urination, weight loss, blurred vision etc, then you may be diagnosed with diabetes mellitus without confirming it with second test.Random Blood Glucose Test:Random blood Glucose test gives your blood sugar at any time in a day. Normal random blood sugar level should be less than 200mg/dl. If your random blood glucose level is between 140mg/dl to 200mg/dl then you will have pre-diabetes.Oral glucose tolerance test:This test measures your response to sugar. First we measure fasting blood glucose level, and then glucose solution is given, after that we measure blood glucose after 1 hour and 2hours. A normal blood glucose level after an oral glucose tolerance test is less than 140 mg/dL. Level between 140 mg/dL to 199 mg/dL suggests pre-diabetes. A blood glucose level of 200 mg/dL or higher two hours after you drink the glucose solution may suggest that you have diabetes mellitus.Glycated hemoglobin (A1C) testThis test is not for diagnosing diabetes, but it shows you how well you have controlled your sugar in last 2 or 3 months. Normal value is less than 7%, however if it is more than 7 then you and your doctor should think of changing your treatment of diabetes.Always Remember, your blood glucose measurement alone is not enough to differentiate between type 1 and type 2 diabetes. Brownies Recipe for the Diabetic

The below recipe is one I have tried, because it is not only quick and easy to make with only five ingredients, but it is the perfect addition to your diabetic diet.  The only sweetness you will find, comes directly from the chocolate pieces and should not have an effect on your blood sugar.

Ingredients:

  • 2 c. fine graham cracker crumbs (24 crackers - organic if you can)
  • 1/2 c. raw walnuts
  • 3 ounce. semi sweet chocolate pcs.
  • 1 c. organic skim low-fat milk
  • 1/4 teaspoon sea salt

Directions:
  1. Preheat oven to 350 degrees. 
  2. Place all ingredients in a bowl and stir until it's blended well. 
  3. Put into a greased and floured 8x8x2 inch pan. 
  4. Bake 30 min. 
  5. Cut into squares while hot and Enjoy.
